Required Health Forms

Medical Information
for Clinic Form

Parents and caregivers must
complete this form annually for
every camper. Please provide
complete information about your
child’s medical history, current
health needs, and medications.
If anything changes after you
submit the form but before your
camper arrives, please update
the information.

Statement
of Health

Each camper must have a
Statement of Health completed
annually by a medical provider.
If your camper requires oxygen,
pulse oximetry monitoring, or
CPAP, these needs must be
included on the form. This
documentation serves to
provideĀ  the specified care
and is required by the
state of Texas.

When Your
Camper Is Sick

Please monitor your child’s
health before camp and do
not send them to camp if they
have symptoms of a contagious
illness, including fever,
undiagnosed rash, vomiting,
or diarrhea.
All campers must be symptom-free for
at least 48 hours before arriving
at camp.

Prescription Meds Requirements

Due to state regulations,

B&B cannot administer the following:

– Expired medications. All medications must be current.
– Medications prescribed to someone else. Medications must be prescribed to the camper only.
– Medications with outdated instructions. If the dosage or instructions have changed please ensure the pharmacy updates the label.
– Medications not brought in the original container. Medications must be in their original container with a prescription label and must be packaged by a pharmacist or manufacturer.
We cannot make exceptions.

Medication Check-In

Please pack all camper medications in a separate bag from the rest of your camper’s luggage. Bring this bag to the Medication Check-In table when you arrive.
For boxed medications (inhalers, EpiPens, and prescription creams), please include the original box with the prescription label or ask your pharmacy to provide an additional label for the medication itself.
NOTE: Please pack only the doses of prescription medication your camper needs for their camp session in the original container.
